About Harbor Well Care Group

More Than Home Care.
A Clinical Partnership.

Harbor Well Care Group was founded on a belief that every person receiving in-home care deserves the same clinical coordination and oversight they would receive in a hospital or skilled facility — delivered with compassion, dignity, and professionalism in the comfort of their own home.

We are not a staffing agency. We are a structured, RN-supervised care organization with documented care plans, mandatory competency sign-offs, state-compliant training protocols, and real-time clinical oversight tracking — for every client and every caregiver, without exception.

Our caregivers complete a pre-employment nursing diploma, 14 required training items, and an RN-supervised competency sign-off on 25 individual skills before they ever enter a client's home independently.

The Story Behind Our Logo

Every Symbol Has
Meaning and Purpose

The Harbor Well Care Group logo is not simply a design — it is a statement of who we are, what we believe, and the promise we make to every client, family, and partner we serve.

🏠
The Lighthouse — A Beacon of Guidance
The lighthouse at the heart of our logo represents our role as a steady guide for families navigating the uncertainty of home care. Just as a lighthouse warns ships away from danger and leads them safely to shore, Harbor Well guides every client and family toward safety, clarity, and confident care decisions.
🌿
The Green Leaf — Growth, Wellness & Compassion
The green leaf that embraces the lighthouse symbolizes life, wellness, and our deeply compassionate approach to care. It reflects our belief that in-home care should nurture the whole person — their health, dignity, spirit, and sense of belonging in their own environment.
The Harbor — A Place of Safety and Refuge
A harbor is not simply a location — it is a promise of protection and stability. "Harbor Well" means we are both the safe haven and the wish for wellness. When a client comes to us, they are entering a relationship anchored in trust, safety, and professional care that holds steady no matter what the seas may bring.
The Circle — Wholeness, Community & Continuity
The circular seal of our logo represents wholeness and the full circle of care we provide. It speaks to continuity — the ongoing relationship between client, family, caregiver, and RN supervisor — and to the community of trust that Harbor Well builds with every person we serve.

Our Care Standards & Policies

Every Caregiver. Every Client.
The Same Uncompromising Standard.

Harbor Well Care Group operates by a comprehensive set of care policies that govern every aspect of the service we provide — from hiring to the moment a caregiver enters a client's home, to ongoing RN supervision and documentation.

🎓
Pre-Employment Nursing Certification
Every caregiver must complete the Alison Diploma in Nursing & Patient Care — a full nursing and patient care credential — before their very first client assignment. Zero exceptions.
14-Item Training Protocol
All 14 required training items — including HIPAA, infection control, abuse prevention, emergency procedures, body mechanics, and fall prevention — must be completed and documented before client assignment.
RN Competency Sign-Off — 25 Skills
The RN supervisor directly observes and signs off on 25 individual caregiving skills across five clinical areas before any caregiver is cleared for independent care. This is the bedrock of our safety standard.
📋
Individualized Written Care Plan
Every client receives a fully individualized written care plan created by our RN following the initial assessment. The plan is reviewed, updated, and documented at every supervisory visit.
🔍
Background & Drug Screening
All direct care staff undergo thorough background screening and drug testing before employment. Exclusion list screening is conducted annually. We accept no shortcuts in who enters a client's home.
📣
Mandatory Incident Reporting
All incidents — falls, near-falls, condition changes, emergencies — are documented within 24 hours and reported through the RN supervisor. Client families and referring facilities are notified promptly.
🔄
Annual Refresher Requirements
Every active caregiver must complete annual refresher training in all required areas — including the Alison course, HIPAA, abuse prevention, and infection control — by December 31st each year. Non-compliance means no active assignments.
📱
Digital Compliance Tracking
Our Clinical & Training Command Center tracks every employee's training status, RN visit log, and competency record in real time. Nothing falls through the cracks — every gap triggers an immediate alert.
Technology & Coordination

A Level of Coordination
No Other Agency Can Match

Our purpose-built Clinical & Training Command Center gives administrators, RN supervisors, and operations staff a real-time view of every employee, training record, RN visit, and incident — ensuring complete compliance at all times.

📊 Real-Time Compliance Dashboard
Every chart and stat updates automatically when data changes. No manual reports needed — the system provides a complete, accurate compliance picture at every moment, for administrators and partner facilities.
⚠ Instant Alert System
Red alerts fire the moment any employee is missing a required certification. No caregiver reaches a client without meeting every pre-employment standard — the system enforces this automatically, every single day.
⚕ Complete RN Visit Record
Every RN supervisory visit is logged with date, type, client tier, findings, and care plan update status. A complete, audit-ready legal record of clinical oversight — for every client in our care, always.
Why Choose Harbor Well

Six Reasons We Are Simply
Different From Everyone Else

01
Active RN Supervision on Every Client
We assign a caregiver AND an RN supervisor who is actively engaged — documenting visits, updating care plans, and serving as a clinical point of contact for families and medical teams throughout the entire care relationship.
02
Mandatory Nursing Diploma Before Day One
Every Harbor Well caregiver completes the full Alison Diploma in Nursing & Patient Care before their first assignment. This is a comprehensive credential most agencies do not require — and we treat it as non-negotiable.
03
RN Competency Sign-Off — 25 Observed Skills
No caregiver is cleared for independent care without the RN personally observing and signing off on all 25 required skills across five clinical areas. This is how we ensure every caregiver is truly ready.
04
Purpose-Built Digital Compliance System
Our Clinical & Training Command Center tracks every employee, training item, RN visit, and incident in real time. Partner facilities can trust that our documentation is current, complete, and audit-ready at all times.
05
Four-Tier Precision Matching System
Clients are matched to a care tier based on clinical complexity — from basic daily assistance to high-acuity behavioral care. Each tier has specific caregiver requirements, RN visit frequencies, and a tailored written care plan.
06
Annual Training Commitment on Every Caregiver
We do not let certifications lapse. Every caregiver must complete all annual refresher requirements by December 31st each year. Any non-compliant caregiver is immediately removed from active assignments until requirements are met.
